Plan and sequence primary and revision facial feminization surgery
The facial feminization surgery course follows James P. Bradley and his team through seven patients, combining 3D CT-based skeletal planning and soft-tissue simulation with brow contouring, hairline advancement, rhinoplasty, cheek work, jaw and chin reduction, lip enhancement, tracheal shave, neck contouring and fat grafting.

The programme
Case workup & virtual surgical design
Each case opens with history, hormone therapy duration where relevant and any earlier operations. Bradley then shows how 3D scan-based skeletal analysis and separate soft-tissue simulations translate a patient’s goals into a region-by-region operative plan.
Upper face: brow, forehead & hairline
Brow contouring and elevation feature in every case, with hairline advancement added in the case of a 25-year-old whose transition started ten years before. The teaching shows how forehead reshaping sets the frame for the rest of the face.
Nose & midface in the facial feminization surgery course
Rhinoplasty for a slimmer profile is shown in primary noses, an ethnically characteristic nose and a revision. Midface teaching includes cheek enhancement and correction of previously placed cheek implants.
Lower face, lips & neck
Jawline narrowing, chin slimming and lip augmentation recur across the series, together with neck contouring and tracheal shave. One patient’s neck had previously been treated with silicone injections, adding a further layer of complexity.
Revision strategy & team workflow
Two secondary cases build on earlier feminization surgery, with supplemental fat grafting to refine contour. Bradley explains his preferred order of interventions, key instruments and how a coordinated operative team keeps lengthy multi-region surgery consistent.
Who it’s for
- Craniofacial and plastic surgeons study combined skeletal and soft-tissue feminization in full cases
- Facial plastic surgeons see how rhinoplasty, lip and neck work integrate into one plan
- Gender-affirming care teams understand surgical planning, sequencing and revision considerations
- Surgical fellows observe 3D virtual planning carried through to the operating room
The consult
Is there a standalone lecture on facial anatomy and theory?
No — the series is case-based. Planning principles and anatomy are explained within each of the seven patient walkthroughs rather than in a separate introductory talk.
Are voice or body procedures included?
No — the focus is facial and neck surgery, including tracheal shave. Voice surgery and body feminization procedures are outside its scope.
How long is each case?
Individual cases run from two hours to more than three and a half hours, giving time for workup, planning and the operative sequence for each region.
Can this course prepare me to offer facial feminization?
It provides detailed insight into one team’s methods, but it does not replace craniofacial training, supervised operating experience or the multidisciplinary care these patients require.
What happens after I pay?
The seven cases are released to your account straight away and stay open to you with no expiry date.
Do I get CME credit or a certificate?
No. Derma Academy provides the course files only — no CME or CE credits, MOC points or certificates of completion are included.
