Blend preservation and structure in primary rhinoplasty, case by case
The Enrico Robotti rhinoplasty videos put you beside the surgeon for five primary open rhinoseptoplasties and one rib-graft revision, showing push-down and let-down dorsal variants, subperichondrial–subperiosteal dissection, ligament reconstruction, septal fusion slings, piezo turbinoplasty and combined piezo and power-tool osteoplasty in every operation.

The programme
Hybrid dorsal preservation, modified dorsal split
Watch high-strip and low-strip strategies applied to different noses, from cartilaginous push down with overlap to full and asymmetric let down. Robotti shows how part of the bony cap can be enclosed and the dorsal surface sculpted to keep a natural profile line.
Structural tip reediting with ligament repair
Tip work combines lateral crural steal, LLC turn-under or slide-under, septal extension grafts and spacer struts. Ligaments are preserved or reconstructed so the tip regains support without the stiff, operated look many primary patients fear.
Planes, swelling control & septal fusion sling
Dissection stays on subperichondrial and subperiosteal planes, and three-point compartmentalization sutures are used to limit oedema and refine contour. Sublaminar septal dissection and the fusion sling suspension credited to Carlos Neves appear in several cases.
Function first in the Enrico Robotti rhinoplasty videos
Severe airway complaints, crooked dorsums and thin skin are handled alongside aesthetics through septoplasty, piezo turbinoplasty and partial middle-vault opening with keystone release. Breathing correction is treated as part of every plan, not an afterthought.
Grafting, alar support & chin balance
A composite conchal graft corrects alar retraction, alar contour grafts refine the rim and diced cartilage softens a tip. One patient also receives an anatomic silicone chin implant placed through an intraoral approach to balance the profile.
Bonus revision with laminated rib
The final case rebuilds a deviated, pinched and mildly saddled nose with harvested rib: L-strut reconstruction, extended spreaders, transposed lateral crural struts and Robotti’s SPLF dorsal graft finished with rib plaster.
Who it’s for
- Facial plastic surgeons compare preservation and structural moves inside one open approach
- ENT surgeons see turbinate, septal and valve work integrated with aesthetic goals
- Plastic and ENT fellows study long, uncut operations with the surgeon’s reasoning
- Experienced rhinoplasty surgeons refine piezo osteoplasty and rib reconstruction
The consult
What kinds of noses are operated on?
The five primary patients include a drooping tip with a wide bony dorsum, residual deformity after childhood neurofibroma excision, two deviated noses with significant breathing problems and a large hump with chin hypoplasia.
Is the bonus case also a primary rhinoplasty?
No — it is a secondary operation on a 41-year-old patient that relies on rib cartilage for structural rebuilding of the septum, tip and dorsum.
Are the operations edited down to highlights?
The footage is long-form, about 21 hours for six cases, so expect complete operative sequences rather than short clips. It complements, and cannot stand in for, supervised surgical training.
Which instruments does Robotti rely on for bone work?
Every case combines piezoelectric inserts with power tools for osteoplasty and osteotomies, and the piezo device is also used for his turbinoplasty technique.
Is there a waiting period before the cases open?
None — the six operations are available right after checkout and stay in your account indefinitely.
Do I get CME credit or a certificate?
No. Derma Academy provides the course files only — no CME or CE credits, MOC points or certificates of completion are included.
