The programme
Academy philosophy & mentoring model
Fernando Nakamura opens with an hour on how the academy approaches hybrid and preservation surgery and how its mentoring community is organised. It frames the later cases around continuous refinement rather than a single fixed technique.
Preservation manoeuvre library
Twenty brief clips from Juan Tommasino each isolate one idea, from high and middle septal strips and bone-cap preservation to subdorsal Z and triangular flaps, Teo suture fixation and the Osmen sliding alar flap. They work as a quick reference before or after watching the full operations.
Open dorsal preservation with Puzzle Tip
The first primary case moves from analysis and incision through radix work, osteotomies, dorsum-to-septum fixation and narrowing, then finishes with footplate sutures and base reduction. Each stage sits in its own chapter for easy review.
Hybrid open rhinoplasty with structured grafts
A second open case blends preservation with structural work: caudal septal repositioning, septal cartilage harvest, graft preparation, cartilaginous roof fixation and tip treatment. PRF combined with diced cartilage on the dorsum is also demonstrated.
Closed SPQR and Spare Roof in the HARM 2.0 rhinoplasty course
Two endonasal operations show an SPQR approach without tip dissection, finished with splints, and a Spare Roof procedure combined with Puzzle Tip plasty and radix grafting. Together they illustrate how much can be achieved without opening the columella.
Structured tip with base reduction
The fourth case preserves the dorsum while building a structured tip, covering bone reshaping, cartilage treatment, osteotomies, alar rim work, tip grafts and staged refinement. Final analysis segments let viewers compare intent with the closing result.
